Physical Medicine and Rehabilitation Center
The
Good Samaritan Physical Medicine and Rehabilitation Center is located on
the main hospital campus at 14th Ave. SE, Puyallup, Wash. The Physical Medicine
and Rehabilitation Center may be accessed from within the hospital or from
either 13th avenue or 14th avenue, SE. For

Rehabilitation entrance
- From Meridian, turn onto 14th Ave. E.
- Pass 3rd Street on your left and turn at the first
driveway leading into Good Samaritan’s parking garage (parking is free).
Follow the sign to rehabilitation parking.
- Drive to the top floor (5th floor) of the garage and
park.
- Enter the hospital through the rehabilitation entrance.
- Patient rooms are to your right. The main nursing station
is down the hall on your right.
- If you are coming to Good Samaritan for an appointment
for rehabilitation services, there is a reception desk in the main rehabilitation
center lobby. You will need to check in at the desk.
Outpatient rehabilitation entrance
- From Meridian, turn onto 14th Ave. E.
- Turn left on 3rd Street, before the parking garage.
- Follow the street around the back of the hospital where
the street becomes 13th avenue.
- There is a loading and unloading area for patients
and families for outpatient rehabilitation on the right, after you have
passed the parking garage entrance.
- You will need to park your vehicle in the lower level
of the parking garage.
- Enter the lobby area and take the elevator either to
the basement level (outpatient rehabilitation) or ground floor (inpatient
rehabilitation).
